Cannot Create Request Template After Creating The JBPM Tables

When attempting to create request template through web access, the following error occurs. JBPM is using as workflow repository and JBOSS is the java application server.

Pop up window error:

'Error: An error has occurred. The template might not have been created'.

Web Access Log Error:
