Probable Cause of Provisioning Interface Error "NACK:69-Could not create ccs_acct_reference entry for MSISDN" (Doc ID 1353296.1)

Symptoms

When running Provisioning Interface (PI) commands to add new subscribers, for an unknown reason, some of the commands fail with the following error message:

<-CCSCD1=ADD:NACK:69-Could not create ccs_acct_reference entry for MSISDN 123456;

When enabling debug on the specific PI command through the interface as in the example below:

debug on CCSCD1=ADD;
CCSCD1=ADD:MSISDN=123456,PROVIDER=Boss,PRODUCT=TEST_PT,LANGUAGE=english,CHARGING_DOMAIN=1,MAX_CONCURRENT_ACCESS=15;
