Goal

When a DCI Layout and DCI Form are generated, the Prov DCI Form Generated? field remained unchecked.

This worked in Oracle Clinical 4.5.3 and Oracle Clinical 4.6.0 when patch 4.6.0.10 was applied for <Bug 9527523> - AFTER GENERATION PROV DCI FORM GENERATED CHECK BOX REMAINS UNCHECKED

This bug has now re-surfaced in OC 4.6.2.  Has this been fixed for 4.6.2?
