Symptoms

Forms or Reports 11g intermittently fails with an "Failure of Server APACHE Bridge" error in the browser or a FRM-92103 error in the form.

To resume normal working, the OHS component in the Forms / Reports technology stack has to be stopped and restarted.

The following errors are logged in <INSTANCE_HOME>\diagnostics\logs\OHS\ohs1\ohs1.log file:
