Forms or Reports 11g intermittently fails with an "Failure of Server APACHE Bridge" error in the browser or a FRM-92103 error in the form.
To resume normal working, the OHS component in the Forms / Reports technology stack has to be stopped and restarted.
The following errors are logged in <instance home>\diagnostics\logs\OHS\ohs1\ohs1.log file:

The error is not specific to Forms / Reports 11.1.2 installations. It can be reproduced also on any Fusion Middleware Installation using OHS (Oracle HTTP Server) 188.8.131.52.0 or 184.108.40.206.0 . If unsure if your version is affected, run <ORACLE_HOME>\OPatch\opatch lsinventory -detail and check the "Patch of Oracle HTTP Server 2.0" version.
At configuration time, the WebLogic cluster option was chosen, and consequently the HTTP Server configuration file, containing the MOD_WL_OHS settings, has the directive WebLogicCluster
In the case of Forms / Reports this parameter is found in the forms.conf and reports_ohs.conf respectively.
