Symptoms
When attempting to to run Data Collections in Production, the following error occurs.

UNIQUE index creation on temporary table SYSTEM_ITEMS_EBS has completed with ERROR.
STEPS
-----------------------
1. Run Collections
2. Note the error in the log file
