Integration Of Insight Is Not Working with Cognos

Symptoms
The standard reports are not accessible when log-in to Insight. Also giving 500 - internal server error when goes to tools|security|Group and access rights tab.
Observed the following errors in the log file:
administrator.err log file:
Error in 'CBITool::Logout 429 - ActiveX component can't create object - Error while user logs out of cognos 8

Error in 'PRBITool.CBITool::GetReportData(1) 0 -
