Unable to Connect to Agile Web Services Due to Failed Attempt to Authenticate to LDAP

Error creating web service session.
Web service client application logs may show the error:
Error message : The account has been disabled. Please contact the administrator.
Root Cause exception : oracle.oc4j.rmi.OracleRemoteException:
com.agile.util.exception.LoginException: The account has been disabled. Please contact the administrator.; nested exception is:
com.agile.util.exception.LoginException: The account has been disabled.
Please contact the administrator.
When attempting to click on Agile Workspace Manager in EC, the login prompt disappears after clicking OK.
Error in the %UserProfile%\AgileCache\a9proxy.log:
Invalid configuration: CAX_USER_NAME not returned
