OpaConfig Page Cannot Be Displayed - HTTP 404 Not Found Error

Goal
Customer is trying to login to the opaconfig page.
The login window pops up and after entering the user details opaconfig/password, a blank page is displayed.
Various errors are encountered when logging in to OPACONFIG using
http://server.domain/onsiteadmin/opaConfig:
ERRORS
----------
"HTTP 404 Not Found"
"The webpage cannot be found".
