Batch Data Load (module RXCBEBLT) Fails when DCM has Multiple Occurences of the same Question / has Performance issue in the 'PREPARE' stage (Doc ID 1458398.1)

Batch Data Load (BDL) will fail if the data being loaded for a DCM has multiple occurences of the same question.  This can happen when the same question is repeated in a DCM question group or the same question group is used twice in
a DCM.

Batch Data Load is also significantly slower after upgrading to Oracle Clinical 4.6.4 from 4.5.3. The following query was identified as being expensive :

select q.que_sub_type_code into :b0:b1  
questions q ,dcm_questions dq ,dcms d where ((((((((q.question_id=
dq.question_id and dq.clinical_study_id=:b2) and dq.dcm_id=d.dcm_id) and
dq.dcm_que_dcm_subset_sn=d.dcm_subset_sn) and dq.dcm_que_dcm_layout_sn=
d.dcm_layout_sn) and and d.subset_name=:b4) and d.dcm_layout_sn=
1) and dq.question_name=:b5)
