With 'Pop Before SMTP' Enabled on the MMP, Why do some SMTP Clients Not Need to Authenticate?

In the Messaging Administration Guide, on 'Enabling POP Before SMTP', in the section 'To Install the SMTP Proxy'  the following is written :

4.Make sure the IP address that the MMP uses to connect to the SMTP server is not treated as “internal” by the INTERNAL_IP mapping table.

We have made sure this is configured correctly but nevertheless we see a certain smtp client does not need the 'POP Before SMTP' process to be able to send mail.
We've checked and it seems the client IP address is inside the INTERNAL_IP range inside the mappings file..

Is this expected ?
