ISE 0x80004005 occurs when submitting the form

Symptoms
One possible cause for getting ISE 0x80004005 when submitting a form could be related to an Oracle error of a value too large.
Error in the event log:
Executing SQL statement DSN: <cdd trial name> Error: SQL Statement Error #1 State: S1000 Message: [Oracle][ODBC][Ora]ORA-12899: value too large for column "<cdd trial name>"."T_<form refname>"."<column using radio control>" (actual: 25, maximum: 11) CODE: -1 SQL: Update t_<form refname> Set <column using radio control>=<selection value of radio control>' where PatientID=<patientid> and VisitID=<visitid> and VisitIndex=<VisitIndex> and ItemSetIndex=<ItemSetIndex> and FormIndex=<FormIndex>.
