When Supplier Opens Electronic Questionnaire (eQ) Receive Error, "The questionnaire is invalid because the source data has changed"

Why does the following error message appear after clicking on an eQuestionnaire link and entering email address and password for an ingredient questionnaire?
"The questionnaire is invalid because the source data has changed"
Steps to Reproduce:
1. eQ is created and sent to the supplier.
2. The supplier enters email and creates a password.
3. Supplier receives error message.
