'Annotate CRF' toolbar button does not appear in the 'File => Create Annotated Layout' PDF Data Entry Window.

Goal
User has installed both the PDF Plugin and Adobe Acrobat (Standard 7.0) on their client desktop machine. After doing so the following behaviour is experienced :
i.
Open the Graphic Layout Editor
ii.
'File => Create Annotated Layout' => opens the PDF Data Entry Window.
iii.
Annotate CRF' toolbar button does not appear on the RDC toolbar (see below screen shot to see what is not appearing) :
