Conference Call: OCSB is Not Sending The CWA

Symptoms
In a conferencing application, while trying to implement the next conference call:
1. First we invite A party with ICA (Initiate Call Attempt)
2. When A party answers, we play announcement, and invite B parties with ICA
3. When one of the B parties answer, we play short announcement to him
4. When announcement to B party finished, we stop announcement for A party, and connect A and B
OCSB is not sending the CWA (Continue With Argument) so MSS (Switch) returns task refused.
