NPA NXX Falls Off From PSR Order Task Maintenance Screen

Symptoms
The NPA/ NXX falls off PSR orders after it is assigned on the Plan Selection tab of the Task Maintenance Screen. The issue is found in the 6.0.16 stream .
Steps to reproduce the issue:
1. Create a new PSR order
.
2. Assign a Prov-plan to it and while assigning the prov-plan populate the npa-nxx field on the same tab above .
3. The order is in View only mode now. Open the task Maintenance window and see that the npa-nxx field is empty now .
Please see the screens below for clarity of the problem .
PSR ORDER.NOTE THE NPA/NXX has been entered .
COMPLETE THE ORDER Or Open the order and go to Task Maintenance screen.NPA/NXX has fallen off.
Expected Behaviour :
When a user opens the Task Maintenance for a PSR , he should be able to see the NPA-NXX field which he entered
