Failed Logins Not Being Recorded in the EVENT_ACTIVITY_VERIFY_T Table (Doc ID 1509411.1)

Symptoms

On all versions of Oracle Communications Billing and Revenue Management (BRM), the EVENT_ACTIVITY_VERIFY_T table stores failed login attempts.  However this table is not being populated despite the following:

1. cm/pin.conf:

Once an error occurs this select returns nothing.
