Symptoms

The PCM_OP_PYMT_COLLECT opcode does not call the PCM_OP_PYMT_APPLY_FEE when the payment is not successful as it should. PCM_OP_PYMT_APPLY_FEE creates payment fees for payments that fail. e.g. due to insufficient account funds or an expired credit cards.
