The CCB-MDM Integration Error Handling Service Failes with Primary Key Violation

When a technical or internal business exception is encountered by the CCB-MDM integration product, the error is persisted to the INTEGRATION_ERROR_STORE table, which is then used by the INTEGRATION_ERROR_LOOKUP table to help highlight and deal with these exceptions. During the persisting of error messages to the INTEGRATION_ERROR_STORE table, the insert fails due to a primary key constraint - which is detailed in the integration layer log file:
The above error message will be a result of the excpetion shown in Enterprise Manager for the ErrorHandling composite, in the Invoke_InsertIntoTable activity.
