Wrong Balance Groups In Migrated Billinfos And Services

Symptoms
When migrating accounts using CMT, the following two issues come up with the Balance Groups it creates.
1) There are 3 billinfos in billinfo_t that are linked to 3 balance groups, but CMT created 4 balance groups in bal_grp_t (2 of these are linked to the same billinfo in bal_grp_t).
2) There are 3 records in service_t, but all of them are linked to the same balance group, even though each service has been linked to different billinfos in the XML.
