Can not View a BLOB Attachment in the WAM Fleet Asset Module - receive error FRM-92101

When a user tries to view an attachment that is stored in the database (BLOB) from a Fleet Asset record, the following error is displayed:
Frm-92101: There was a failure in the forms server during startup
The problem only occurs in the Fleet Asset module. The attachment can be viewed from the Document Control module and the attachment view of other modules.
The issue can be reproduced at will with the following steps:
1. Create a document that is stored in the database in the Document Control module
2. Add the Document record to the Attachments view of the Fleet Asset module
3. Select the View button
4. Error is displayed
