MMP Pop Or IMAP Proxy Unexpectedly Closing The Connection "Error Connection Limit Reached For Your IP Address" (Doc ID 1585402.1)

Symptoms

We are using Oracle Communications Messaging Server 7u4-25.01(7.0.4.25.0) 64bit (built Feb 29 2012).  Our topology consists of the following:  two front-end servers for MMP, MTA and Web; two additional front-end servers for Convergence and Delegated Admin; two back-end servers for the message store, and two other back-end servers for LDAP.

When Outlook users try to connect to the Messaging Server, they see the following error in a popup:

On the MMPs, the following errors were observed:

default log:

[22/Aug/2013:10:00:31 -0500] mmp1 immonitor-access[6212]: General Notice: Timed out after 30.042 seconds
[22/Aug/2013:10:02:31 -0500] mmp1 immonitor-access[6675]: General Notice: <POP> for <IP address>:110 timed out
[22/Aug/2013:10:02:31 -0500] mmp1 immonitor-access[6675]: General Notice: Timed out after 30.044 seconds

PopProxy.log:

[22/Aug/2013:10:02:56 -0500] mmp1 PopProxy[2656]: General Error: (id 1568736) Connection limit reached for client IP <IP address>


We restarted all of the MMP services and after that, the above errors went away.  What is causing this issue and how can we prevent it from occurring?
