Unable to import the follow-up ICSR file using the PMDA profiles

Symptoms
The following error appears when accept the follow-up ICSR under the Reports > Incoming E2B reports.

Steps:
1. Navigate to Reports > E2B Pendings (Incoming E2B reports)
2. Select the follow-up ICSR record which is using the PMDA (-I and -J) profiles
3. Accept the ICSR file
4. Enter the password with the justification text
5. Click OK
6. The following error appears in the dialog box

Note:
In the Utilities > Logs > View Error Log (CMN_ERROR_LOG), the following error text has been stored.
Argus Safety.COM :- Oracle error 4116 occurred.
OIP-04116: Cursor not ready for I-O
