Reset Password Link Returns Error - Failed To Reset Password (Doc ID 1603219.1)

Goal

The following error is encountered when clicking on the Reset Password link highlighted in red at the bottom of the RDC Onsite login form.

ERROR
------------
Failed to reset password. Please contact your help desk.

Error is encountered when the user enters the name in the User Name field and then clicks on Reset Password button.

What could be the cause of this?
