Symptoms

Order failed during submission with the following error:

Failed to create and start the order due to java.lang.RuntimeException: com.mslv.oms.OMSException: encountered error starting orchestration caused by:Predecessor Order has not been created, reject follow on order.

However, sometimes the order is successfully submitted but it does not trigger any tasks in OSM.
