Symptoms

When trying to log into Oracle Clinical with a newly created Oracle Account,
the following errors are received:

ORA-04067 not executed, package opa.opa__user_status_bar not executed
FRM-40735 WHEN-NEW-FORM-INSTANCE trigger raised unhandled exception ORA-04067.
