Symptoms

When attempting to select a study from a List of Values (LOV), you noticed there are two (duplicate) entries for the study. Once the study is selected, the following error occurs:

ORA-01422: exact fetch requested returns more then requested number of rows
ORA-06512: @"RXC.RXC_validate_pack", line 136
