Unable To Login Into Insight Login Page -login failed 3 times (Doc ID 1643068.1)

Symptoms

Unable to login into Insight as "Administrator" user.
This is a new install.

"login failed 3 times.
Your account has been disabled. Contact your administrator"

When trying to login as any other user [which got created via DB link] - similar error appears.

Changes

 Check the cfg_users table and make sure the administrator user is not disabled.
