Stop date appears on the PSUR same as start date even though it is not entered in the case

Treatment Dates Appear on the PSUR Incorrectly.
Steps to Reproduce
- Open a case
- Add Vaccine information in Vaccine tab with a start date.
- Save the case.
- Check the details saved in the back end table CASE_DOSE_REGIMENS
- Note that stop date is getting updated with same date as start date
- PSUR report is displaying the same stop date even though it was not entered by the user.
