Inform Adapter ODM - "The Security Token could not be Authenticated or Authorized" Client error

When attempting to extract data via the ODM Export interface for a particular trial (specifying any method), under username/password authentication, the error "The security token could not be authenticated or authorized" appears. Other trials on the same adapter but different ServerAdapters work fine.

Using SOAPUI, the SOAP response looks like this:

         <faultstring>The security token could not be authenticated or authorized</faultstring>

Using ODMSampler, the user just gets a popup message stating "The security token could not be authenticated or authorized".

The web log shows the activity with 500 status codes but no errors appear in the Adapter server event log.
