Unable to Add File After Applying Patch 9.3.3.0.30 and 9.3.3.0.61 on Clustered Environment (Doc ID 1902071.1)

Symptoms

When attempting to add file after applying patch 9.3.3.0.61 (Note that prerequisite patches: 9.3.3.0.2, 9.3.3.0.3, 9.3.3.0.15, 9.3.3.0.30 are applied also) on clustered environment, file fails to load with "Error" in the client screen.
The following error occurs in tomcat localhost log file:

STEPS

The issue can be reproduced at will with the following steps:
1. Apply patch 9.3.3.0.2, 9.3.3.0.3, 9.3.3.0.15, 9.3.3.0.30, 9.3.3.0.61 to Application Server
2. Login to Web Client, and add file
