CCR Terminated Without Terminating Call

Symptoms
When making a long call (~2 hrs), found that Oracle Communications Network Charging and Control (OCNCC) has terminated the charging while the call isn't terminated on the network. From the trace, it seems Oracle Communications Service Controller (OCSC) suddenly stops responding to ApplyCharging from OCNCC (that's why OCNCC terminates the charging), and it is not sending out ApplyCharging to the network.
