Some values (A.1.4 / B.1.2.2.x / J.8) are not populated in PMDA paper report (Doc ID 1942257.1)

Symptoms

When printing the PMDA paper report form-1/2,

some values such as A.1.4 "Report type", B.1.2.2.x "Patient Age" and J.8 "New Drug Class Category" are not populated in the report even though the data exists in the case.
