"The server has encountered an error. Please try again later" When Logging Into Convergence

Symptoms
Using: Convergence 3-0.01, Glassfish 3.1.2.8.
We followed the Logging User IP Address and Session Tracking Information and we have a GlassFish Server hosting Convergence that resides behind a front-end reverse proxy or load balancer. So, because of this, we need to set the authPassthroughEnabled parameter to true on the GlassFish Server.
We are using the following command for Glassfish 3.1.2.8, which appears to work fine:
# /opt/sun/glassfish3/bin/asadmin set server-config.network-config.protocols.protocol.http-listener-1.http.auth-pass-through-enabled=true
server-config.network-config.protocols.protocol.http-listener-1.http.auth-pass-through-enabled=true
Command set executed successfully.
But, after doing so, some users who access Convergence by bypassing our proxy/LB are unable to access Convergence and receive an error of "The server has encountered an error. Please try again later" when trying to log into Convergence.
At the time of the error, the following error is observed in the iwc.log:

Upgrade from Convergence 2p7 and Glassfish 2.x to Convergence 3-0.01 and Glassfish 3.1.2.8.
