Single User Receiving FRM-40735 / ORA-01403 / ORA-06512 Errors When Clicking Submission Tracking

 On Applications Oracle Adverse Event Reporting System Version 4.6.2

A single user (USERA) is receiving FRM-40735 / ORA-01403 / ORA-06512 errors when he clicks on some submission tracking (ST's) for various cases in AERS 4.6.2

If log into AERS on his computer as USERB account or another account, can open the EMEA ST's on the case fine.
If  log into AERS on my computer, I using USERB can open the EMEA ST's also.

Can access other EMEA Submission Tracking rows on other cases.


The issue can be reproduced at will with the following steps.

 1. Log into AERS as affected user
 2. Submission Tracking, click on submission tracking records.
