Error in Chain Maintenance Screen: 'FRM:40508 -ORACLE error: unable to INSERT record'

Error: "FRM:40508 -ORACLE error: unable to INSERT record." occurs when trying to create an already existing area on Area Maintenance screen.
Steps to Reproduce:
- Log in to RMS front end.
- Navigate to Actions--> Organization Hierarchy--> Chain
- Create a new Chain with the help of New button.
- In Area Maintenance screen, try creating an already existing area by giving the Chain number and clicking on OK button.
a. Error pops up "Chain number already exists. Enter a new value.
b. Unable to save changes to database .If necessary contact appropriate support personnel.
Now, if we are trying to create a new area by giving a unique number and clicking on OK button:
a. It should prompt for other mandatory fields to be entered like "Area description"
a. Getting the error: "FRM:40508 -ORACLE error: unable to INSERT record."
b. Unable to save changes to database. If necessary contact appropriate support personnel.
