Unifier Log In Screen Stuck On 'Connecting To Unifier Database', error 'Create cluster lock' displayed in Unifier command prompt window

After deploying Unifier R15.1 and attempting to sign on for the first time, the Unifier login screen displays a 'Connecting to Unifier database' message and user is unable to type in the username text box. The focus keeps moving to the password box and the log-in button is disabled. The Unifier server displays a 'Create Cluster Lock' error in the Unifier command prompt windows and errors are reported in the Unifier log.
STEPS TO REPRODUCE
NOTE: To reproduce problem, the database account that ran the grant and create script does not have proper prvileges or a commit to database was not performed.
1. Login Unifier 15.1
2. A message "Connecting to Unifier database..." in Unifier login screen
3. The user is unable to type in the username text box; the focus keeps moving to the password box and the log-in button is disabled
4. Check the Unifier application log, to see messages similar to below:
...~ [Unifier Startup] ~INFO ~ taskdefs.SqlBuilderTask ~ ~ processSql for TABLE: sys_server_lock ACTION: create~
...~ [Unifier Startup] ~INFO ~ util.SqlUtils ~ ~ About to execute SQL: INSERT INTO sys_server_lock VALUES ('cluster')~
5. Check the Unifier command prompt window to see message 'Create Cluster lock ...'
