IPSA Unable To Login To A Juniper Router

Information in this document applies to any platform.
IPSA is unable to login to a particular Juniper router, even though the login is via TACACS and set the same for all Juniper routers in the Domain.
We see the following error messages:
20150414-071309|0604|CRT0| Throwing Common::Transaction::CommitFailed( 'Could not login to the device' )
