All lab data with partial date does not populate in PMDA Paper report form-2 page3

Symptoms
If there is multiple lab data which have the same partial date on the same test name,
only one of the lab data is populated in PMDA Paper report form-2 page#3 so all of the lab data does not display here.
Steps to reproduce:
1. Open the new case and go to the patient tab
2. In the lab info section, enter the multiple lab data which have the same partial date on the same test name.
eg.
Test Name: AST -- Test date: 2015/04/?? --- lab results: 100
Test Name: AST -- Test date: 2015/04/?? --- lab results: 200
3. Enter the mandatory value in order to print PMDA E2B/Paper report
4. Save the case
5. Schedule PMDA E2B
6. Click the draft link
All of the lab data are populated properly in B.3.1 <test>.
7. Change the view format to PMDA Paper report
8. Check the form-2 page#3.
