Action Queue Viewer Lists an Action Updating a Category for the Portfolio Named "All Projects", but no Such Portfolio Exists in the Database.

On OPPM version 9.1.1, when viewing the Actions Queue viewer:

A single action appears in the Actions Queue Viewer without apprently haveing been triggered by a user.  The details for that action indicate that is is for a Portfolio called "All Projects", which does not exist in the customer implementation.

Actions in the queue should not appear spontaneously and should be linked to objects that exist in the implementation.
