'internal error' appears when updating a 'custom review' CRF (Doc ID 2039731.1)

Symptoms

1) The following error appears in the data entry window when updating and saving a 'custom review' CRF :

Error
The data entry window has encountered an unexpected error and
will shut down. If the problem recurs, please report the following
information for diagnostic purposes:
Log code xxxx, internal error.

2) The generated dump file contains :


#Starting dump of context parameters for failureId: 7531953812

Exception Details:IO Exception. Unable to proceed further.

3) The following error is seen in Oracle Clinical when updating and saving the same 'custom review' CRF :
