Save Button Not Enabled after Uploading Document(s) and forced save failed with error: Operation Failed System Error (Doc ID 2040508.1)

On: OPPM 9.1.1 version

When attempting to save an updated OPPM Form without having filled in mandatory fields, the following error occurs.

Operation Failed - System Error!
System encountered an error, please contact system administrator with reference id=457e148b-2e76-4d45-b1f3-82facd3d2266

The issue can be reproduced at will with the following steps:
1. Login to OPPM and select one item on form tab.
2. Upload the document without filling in any category on form.
3. After upload, Save button is not enabled.
4. After adding some text to any Category, Save button gets enabled.
5. After clicking Save button a warning message pops up.
6. After clicking OK to force Save on Form, the Error occurs.

The issue has the following business impact:
Due to this issue, users cannot save the update in the form.
