CRF Annotation Tools Not Found Using Adobe Acrobat 10 (Doc ID 2040979.1)

Goal

Customer has installed version 4.6.0.29 of the OPA Plug-in with RDC 4.6.6, confirmed Adobe Reader is not installed and using Adobe Acrobat Pro 10, with IE 9.
However, CRF Annotation Tools are not found using Adobe Acrobat 10. The RDC Toolbar is not displaying.
How to resolve this?
