Special characters in Notifications and Comments are being replaced by HTML (hex) entities in output

Unreadable characters show in object History tab
Special characters: "?", "!", ";", "(", "/", " or ", ":", "[", " ' "

Steps to reproduce:
1. Create a change with a workflow assigned
2. Change the workflow status and add any special characters from listed above in the Comments field
3. Review the change's history tab or notifications
