Inconsistent Behavior of JCA Adapter in Production HA Environment

On Oracle Communications Billing and Revenue Management(BRM), 7.5 PS10 version, Webservices Mgr & JCA Adaptor
Inconsistent behavior of JCA apter is observed in production HA environment.
Here the user have applied the <Patch 21576490> for the JCA adapter by enabling the global transaction flags and the same has been deployed in production environment. The issue is from CRM placed about 10 orders, then for few orders JCA is behaving as no transaction and other few it has a behavior of global transaction, why?
