Symptoms

On : 7.3.3 version, Form Rules

When attempting to run Report - GRC Controls Form Rules
the following error occurs.

ERROR

APP-FND-00806: The default value * is invalid for the P_USER_FORM_NAME segment. Your default value will be ignored. Please inform your system administrator that:

Add the value * to the value set for segment P_USER_FORM_NAME, or choose another default value in the Key Flexfield Segments form or Descriptive Flexfield Segments form. Use the Edit Concurrent Program or Define Report Set form for Standard Report Submission report defaults.

APP-FND-01564: ORACLE error 1422 in FDFGVD

Cause: FDFGVD failed due to ORA-01422: exact fetch returns more than requested number of rows
ORA-06512: at "APPS.LA_GET_USER_FORM_NAME", line 5.

The SQL statement being executed at the time of the error was: SELECT USER_FORM_NAME,USER_FORM_NAME VALUE, USER_FORM_NAME DESCRIPTION, NVL('N', 'N'), NVL(TO_NUMBER(NULL), -1), NULL, NVL('Y', 'Y'), NVL(TO_CHAR(TO_DATE(NULL), 'J'), 0), NVL(TO_CHAR(TO_DATE(NULL), 'J'), 0) FROM (SELECT '*' USER_FORM_NAME FROM DUAL UNION SELECT DISTINCT LA_GET_USER_FORM_NAME(LA_BR_RULE_ELEMENTS.FORM_NAME) USER_FORM_NAME FROM LA_BR_RULE_ELEMENTS) WHERE 1=1 AND USER_FORM_NAME = :X and was executed from the file &ERRFILE.
