On Oracle Communications Network Charging and Control (NCC) release version 5.0.2, the CCS_RES_LMT_ACCT_PREFIX_UQ Index is erroneously installed in the USERS tablespace:

The above index should be installed in the CCS_INDEX tablespace.

Upgrade to or installation of NCC 5.0.2.
