Symptoms

When attempting to modify Change Order the following error occurs randomly:

ERROR

ORA-00001: unique constraint (AGILE.CHANGE_HISTORY_PK) violated.


STEPS TO REPRODUCE

1. Logon Agile WebClient as any user.
2. Open and modify any change order.
3. Will see error randomly on WebClient User interface.
  ORA-00001: unique constraint (AGILE.CHANGE_HISTORY_PK) violated
