Oracle Clinical Layout Editors not working after applying TMS 5.1.1.0

Symptoms
In Oracle Clinical, the Layout Editors are failing to open. The normal pop-up message which follows appears however the layout editors never open.
The issue is affecting both the Graphic and Character Layout editors.
Steps to reproduce the issue:
a). Login to OC -> Definition -> DCMs -> DCMs ->Press on "Special" and select "Edit Character Layout" => Character Layout is not opening
Or
b). Login to OC -> Definition -> DCMs -> DCMs ->Press on "Special" and select "Graphic Layout" -> Edit => Graphic Layout is not opening.
Changes
TMS Patch Set 5.1.1 (Patch 21553712: Oracle Thesaurus Management System 5.1.1 ) was applied without error.
