When attempting to open PMDA ICSR Validation Report, the following error occurs.

ERROR

The SGML could not be generated for parent element:
MHLWADMICSRREMARKS and child element:
MHLWADMICSRREMARKS4. The following error occured:
ORA-01422:exact fetch returns more than requested number of row s.
Stored Procedure: GENERATE_SAFETYREPORT

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create a case form.
2. Enter all PMDA required fields.
3. Schedule and generate a PMDA report.
4. Open PMDA ICSR Validation Report.
