On :  8.2.0 version, Core System

Reference number missing  

A new case is created but the case reference number is not displayed in the title bar; OR the case reference number is deleted and now the case has no reference number.  This is an intermittent issue, happening a couple times a year.
When a case is opened, the case reference number should be displayed in the case entity title bar.  In some agencies, this number is then used to attach scanned documents to the case.

The issue has the following business impact:
Due to this issue, users are unable to attach necessary documents to the case/client.
